Intake fee: $50.00

Level I Fee: $150.00 (total for all three level I classes required, not including intake fees)

DUI Level II Education & Therapy / EOP

Intake fee: $50.00

Book fee (each for education book and therapy book): $30.00

Level II Education (per 2-hour weekly session): $30.00

Level II Therapy (per 2-hour weekly session for Track A, B, C, or D and/or EOP): $30.00

Evaluation

In-State Evaluation: $200.00

Out of State Evaluation: $250.00

Non-DUI (Cognitive Skills/ Relapse Prevention/ Anger Management)

Intake: $50.00

Cognitive Skills workbook: $30.00

Relapse Prevention workbook: $20.00

Individual Session (each 1 hour session): $40.00

Other

Urine Analysis fee (per each 8 panel test with ETG): $20.00

Re-enrollment fee (for all discharged and returning clients): $50.00

Follow Up Assessment (if applicable): $25.00

Interlock Enhancement Counseling (per month, 2 hours credit/month): $30.00

Additional copy of a DRS, Evaluation, or Certificate (after 2 free copies): $5.00

Appearance/Testify in court (1 hour including prep time): $100.00
